Research Consent Document
Amvet Biosciences aims to make and support meaningful scientific discoveries by enabling its participants to participate directly in research conducting in our laboratories for the development of new treatment and diagnostic methods for genetic disorders of animals. We invite you to take part in this project by allowing our researchers to use your animal’s genetic information for research. Your participation in the research study is completely voluntary.
We promise to protect your privacy and treat the information you give us as confidential according to the privacy policy stating in the following. The information you provide will be used only for research purposes only. Your decisions about participating in a study, responding to specific questions, or discontinuing participation will be respected without question.
Amvet Biosciences services are for research, informational, and educational use only. We do not provide medical advice. Our services are not intended to be used as a substitute for professional medical advice. You should always seek the advice of your veterinarian with any questions you may have regarding diagnosis, cure, treatment, mitigation, or prevention of any disease or other medical condition or impairment or the status of your animal’s health.
Based on the results of this research study, Amvet Biosciences may develop intellectual property, including but not limited to patents, copyrights and trademarks, and/or Amvet Biosciences may commercialize products or services, directly or indirectly. In such cases you will not receive any compensation.
Privacy Statement
1. Purpose
The aims of the research conducting in our laboratories are to identify the prevalence of disease genes in different breeds and develop gene therapy in order to develop new and effective treatment and diagnostic method for genetic disorders of animals.
2. Collection
We obtain information of you and your animal from you when you order genetic services with us. We may also receive information about you and your animal from third parties, for example from our service providers or from a friend who wants to tell you about our genetic services.
3. Information
The information we collect might include date of birth, breed, coat color, gender and genomic information of your animals. Any other information you provided when you order our services will not be collected. Wherever possible we use aggregated or anonymous information, which does not identify individual animal.
4. Use
We will only use and disclose your animal’s information for the purpose of conducting the research study and in accordance with this statement. We will not use or disclose your animal’s information for the purpose of advertising, promotions or direct marketing activities.
6. Disclosure
We will not disclose any personally identifiable research information we collect from you and your animal unless we have your express prior consent and will only report the information you provide in an aggregate form that will not personally identify you or your animal.
We will not disclose any personal information or personally identifiable research information to a third party for a purpose other than conducting the research study unless we have your express prior consent or are required to do so by a law or court/tribunal order.
In the course of conducting the research study, we may rely on third party service providers to host or store the data we collect. We take all reasonable steps to ensure that third party service providers comply with this privacy statement.
7. Openness
You have the right to request access to any personal information we hold about you. You can request this information by contacting Amvet Biosciences. Where we hold information that you are entitled to access, we will respond to your request in a reasonable time and endeavor to provide you with a suitable range of choices as to how access is provided.
If at any time you believe that personal information we hold about you is incorrect, incomplete or inaccurate, then you may request amendment of it and we will either amend the information or make a record of your comment, as we think appropriate.
8. Complaints
If you have any questions about this statement or believe that we have at any time failed to keep one of our commitments to you to handle you and your animals’ information in the manner required by this agreement, then we ask that you contact us immediately.
9. Data Disposal
Amvet Biosciences will destroy or de-identify your and your animal’s information as soon as reasonably practicable once it is no longer required to complete the research project for which it was collected. However, we may in certain circumstances be required by law to retain your and your animal’s personal information after our research has been completed. In this case your and your animal’s information will continue to be protected in accordance with this document. If we destroy personal information we will do so by taking reasonable steps and using up-to-date techniques and processes.
10. Miscellaneous
This policy is effective from the date of publication. This policy will be reviewed periodically. Although we intend to observe this statement at all times, it is not legally binding on the Amvet Biosciences in any way. From time to time we may regard it as necessary or desirable to act outside the policy. Amvet Biosciences may do so, subject only to any other applicable contractual rights you have and any statutory rights you have under the Personal Data (Privacy) Ordinance or other applicable legislation.
